To Request a Trial:

Fill out the back of the original ticket, or in the absence of the original ticket, download and fill out the Notice of Intention to Appear.

This document must be signed in ink, and turned in to the Gananoque Police Service for submission to the Provincial Court. Forms can also be filled out at the station.
